By submitting this form, you are agreeing to be billed by StoneBridge School for the service amount you select. I/We herewith notify StoneBridge School that my child(ren) will enroll for the 2025-2026 school year to use Extended Care as noted on this form. In consideration of said enrollment, Parent/Guardian agrees to the following: Past-due Payments and Delinquent Accounts: If standard fees become past due by 60 days, the student may not attend extended care until the account is brought current. Whether a student is withdrawn or dismissed, the fees obligation shall remain with the parent/guardian and the parent/guardian agrees that this covenant shall survive such events. Please note the provisions of this paragraph do not apply to payments made by EFT that are drawn during the month the payment is due.
